1. Follow organisation and chairing approach as defined in Stage 3.

2. Ensure the adult continues to be involved and has appropriate representation as required.

3. Partner agencies share any new information relevant to the case.

4. Agree an updated risk assessment and ongoing action plan.

5. Review the risk assessment and update assigned actions as needed.

6. If the action plan has not reduced risks to an acceptable level, explore whether other flexible or creative options may be required.

7. Agree ongoing monitoring and review arrangements to ensure risks are managed appropriately over time.
