Concerns can be raised by anyone using the Safeguarding Adult Concern Referral Form.
If the concern is about someone within Devon local authority boundaries please raise your concern via this online form. Guidance on using the online form can be found here. If the link to the online form is not accessible the Word version of the form can be downloaded and sent to Care Direct via email@example.com
Concerns can also be raised by telephone, email, or letter.
