Report your concern

If you report a safeguarding concern you will be listened to, supported and involved in any decisions.

In an emergency always call 999

Concerns can be raised by anyone using the Safeguarding Adult Concern Referral Form.  

If the concern is about someone within Torbay local authority boundaries please raise your concern via this Word version of the form and return the completed form to safeguarding.alertstct@nhs.net

If the concern is about someone within Devon local authority boundaries please raise your concern via this online form. Guidance on using the online form can be found here. If the link to the online form is not accessible the Word version of the form can be downloaded and sent to Care Direct via csc.caredirect@devon.gov.uk

Concerns can also be raised by telephone, email, or letter.

If you are in Devon call Care Direct on 0345 1551 007 or email csc.caredirect@devon.gov.uk (Care Direct is open Monday-Friday 8am-8pm and Saturday 9am-1pm). Outside of these hours or on bank holidays call the Emergency Duty Service on 0345 6000 388 or email)

If you are in Torbay call 01803 219700 or via email – safeguarding.alertstct@nhs.net

If you are in Plymouth call 01752 668000 or via their online form.
